Hereford High School offers every student an opportunity to participate in a wide variety of athletic programs. Our forty-seven team interscholastic athletic program is considered to be one of the most competitive and diversified in Maryland.  You may browse our sports schedules by clicking on a sporting event listed on the left of this webpage, or check out the documents and forms below for more information on joining a sports team.


Registering for a Sport - FormRELeaf

All student/athletes must be registered on FormReLeaf prior to participating.  Go to www.formreleaf.com and sign in for an account. All required information, agreements, and forms on Formreleaf must be completed online prior to trying out for a sport. Parents will also complete the medical history page of the Preparticipation Physical Exam form, print it, and bring the form to a physician for the physical examination to be completed by the physician. Athletes must bring their completed preparticipation physical exam forms on the first day of practice and submit directly to the coach. With the exception of the physical form, all other required forms must be completed online and do not need to be submitted to the coach. Do not mail physical exam forms to the school!

   Pre-Participation Provider Form
This form needs to be filled out and signed by a healthcare provider before a student can participate in athletics at Hereford High School. The evaluation is good for one year.

Pre-Participation Parent Form
This form needs to be filled out and signed by a parent/guardian before a student can participate in athletics at Hereford High School. The evaluation is good for one year.

Return-to-Play Form
This form needs to be filled out and signed by a healthcare provider before a student can return to play after having a COVID-positive test.
